Customer Data Sheet:
It is very important that your Customer Information Sheet is current. This informs us on who and how to get a hold of someone if there is an emergency and/or fire at your property.
- Customer acknowledges that it is his/her responsibility to report any deficiencies or troubles on the fire alarm and/or fire sprinkler systems to Kodiak Fire Protection, Inc.
- Customer acknowledges that it is his/her responsibility to keep their customer data sheet updated.
- Customer acknowledges that it is his/her responsibility to provide 24-hour fire watch service on the property until the fire alarm and/or the fire sprinkler systems are operable.
- Customer acknowledges that Excessive Signal Occurrence (Run-Away Signal) notification must be resolved ASAP at the subscribers expense and will be billed at $1.00 per signal after 100 signals have been received in a 24-hour period.
